How to fix hollow temples
a softer, balanced upper face
Hollow temples are a sunken area at the sides of the forehead, caused by fat and bone changes with age or weight loss. Restoring volume with filler or a collagen stimulator can soften shadows and make the face look more youthful and rested.

How do you fix hollow temples without surgery?
The two main options are hyaluronic acid temple filler , which restores volume immediately and typically lasts around 12–18 months, and Sculptra, a PLLA collagen stimulator that builds fullness gradually over 2–3 sessions. Because temple hollowing often pulls the outer brow down, some patients also benefit from a PDO thread lift. See our dermal fillers page for how we keep results natural.
A small area that changes the whole face
The temples are one of the first areas to lose volume, and a dip here can make the face look older, thinner or tired.
Fat loss with age
The fat pads under the temple thin over time, leaving a concave shadow beside the brow.
Bone changes
Gradual changes in the skull and brow bone reduce structural support at the sides of the face.
Weight loss & exercise
Significant weight loss or very low body fat often shows first in the temples and cheeks.
Genetics & face shape
Some people naturally have narrower temples, which can make the upper face look angular.
Treatments we use for hollow temples
Your doctor selects products and depth based on anatomy and how much volume is needed:
- Hyaluronic acid temple filler – immediate, adjustable volume with a cannula or deep injection
- Sculptra (PLLA) – stimulates your own collagen for gradual, longer-lasting fullness
- Supporting fillers – small amounts in the brow tail or mid-cheek for overall balance
- PDO thread lift – lifts the outer brow and reduces shadowing
- Rejuran and skin boosters – improve thin skin quality over the temple
A smoother upper-face contour
What to expect at your visit
Upper-face assessment
We look at temple depth, brow position, cheek volume and skin thickness.
Product choice
Your doctor explains HA filler versus Sculptra, amounts needed and costs up front.
Careful injection
The temple has important vessels, so we inject deep or with a cannula, using numbing for comfort.
Review & top-up
Filler settles within about 2 weeks; Sculptra builds over months, with sessions spaced 4–6 weeks apart.

Aftercare & benefits
- Mild swelling, tenderness or chewing discomfort may last a few days
- Avoid pressing or massaging the area unless instructed (Sculptra has its own massage routine)
- Skip saunas, heavy exercise and alcohol for 24–48 hours
- Contact us promptly if you notice unusual pain, blanching or vision change

Hollow temple questions, answered
Is filler or Sculptra better for hollow temples?
HA filler gives immediate results and can be dissolved if needed, so it suits people who want a quick, adjustable change. Sculptra works gradually by stimulating collagen and is often chosen for a longer-lasting, very natural build-up. Some patients combine both.
How long does temple filler last?
Typically around 12–18 months, as the temple has little movement. Duration varies with product, amount and metabolism.
Is temple filler safe?
Temple filler is a common procedure, but the area contains blood vessels, so it must be done by an experienced doctor using careful technique. Your doctor will explain the risks, including bruising and rare vascular complications.
Does temple filler hurt?
Most people describe pressure rather than pain. Fillers usually contain lidocaine, and numbing cream can be applied first.
Will fixing my temples lift my brows?
Restoring temple volume can subtly support the outer brow and reduce a tired look, but it is not a surgical brow lift. Threads or Botox may be added for more brow lift.
